Obsevership means you go to the hospital and just observers the residents and attendings.Externship is experiential learning opportunities offered by educational institutions to give students short practical experiences in their field of study.The purpose of doing observership/externship is as follows

It increases the  chance of getting residency around 10-20%

Since there is lot of competition among foreign medical graduates and if you do observership /externship,program directors may prefer you. Some community programmes really like candidates who did observership/externship

You can show the programme that you are really interested in residency and you are very enthusiastic to work in USA

You get acquainted with health care system in USA and have some thing to talk about when you go for interviews.

Unfortunately, the list of the hospitals offering observership/externship is decreasing every year. Some graduates try to do observership/externship in doctor's private office which is better than doing nothing but observership/externship from hospitals which have residency programmes is preferred. The best way to find out the hospitals to do observership/externship is contacting them and it is painful process but it helps you to get residency.
